What should I expect on my first visit?All new or returning patients will be asked to fill out paperwork pertaining to their information, insurance, and clinical history. Once the doctor reviews the paperwork, they will conduct a history intake and an exam. An exam consist of range of motion, orthopedic test, neurological test, and specialty test. Our goal is to find the source of the problem. After that, the doctors will discuss their exam outcomes and the next steps. Then treatment will done the same day. What paperwork should I bring?Please bring to your visit: - your insurance ID card - your photo ID - method of payment for any out of pocket expenses…(i.e. copays , coinsurances, deductible, self-pay balances) - If you had any imaging performed, please bring a copy of any imaging reports

What if I don't have insurance or participate with your current insurance plans?Payment for services is expected at the time of your visit. Please call our office to obtain our current fees. We accept cash, check, credit card for methods of payment.

I received a notice that my insurance company has paid on my bill - but I can't understand how they calculated their payment amount. Do you know?You should call your insurance company first and inquire with the customer services department. If you still have questions after speaking with them, you could call our office for further guidance.

Why am I getting bills from physicians?You could get a bill from us if a payment on a deductible, copay or coinsurance was not paid at the time of visit. You could also be getting a bill if your insurance company has denied your claim. Please call our office if you aren’t sure why you are receiving a bill from us.

What billing or insurance information will I receive?You will not receive any billing or insurance information from us unless it is in reference to a balance due bill. Information sent to you by insurance varies from insurance company and the way you receive it may vary too.
